expect as a Clinical Transformation Manager, supporting Social Work
and Population Health at DaVita:
- To be part of a fun, energized team of caring individuals
- Work to advance the role of Social Work
- Collaborate across teams to drive patient outcomes
- Working with senior level leaders to advance strategic
prioritiesResponsibilities include, but are not limited to:
- Will manage multiple projects across the National SW and
Integrated Kidney Care (IKC) teams; proactively communicate project
updates to all stakeholders and maintain a deep understanding of
team metrics and goals, to help inform strategy for the team
- Drive strategy across lanes to improve patient health
outcomes
- Develop and deliver training to appropriate teammates related
to depression
- Partner with Social Work and Integrated Kidney Care teams to
develop workflows that align with strategic priorities
- Leverage new and existing workflows to facilitate Integrated
Kidney Care/DaVita Kidney Care collaboration
- Partner with various members of the National Social Work Team,
to develop training for field social work teammates, on their role
in value based careEducation and Qualifications:
- MSW, LCSW preferred
- Experience in a behavioral health setting providing evidence
based interventions to improve patient outcomes
- Knowledge and experience diagnosing behavioral health
conditions
- Experience working on a multi-disciplinary team, in a
value-based environment, to support population health
initiatives
- Minimum experience: 3-5 yearsHere is what you can expect when

Range$68,400.00-$100,400.00 / yearCompensation for the role will
depend on a number of factors, including a candidate's
qualifications, skills, competencies and experience and may fall
