(SLP) Overview: The Speech Language Pathologist (SLP) is
responsible for delivering rehabilitative speech therapy to
individuals in skilled nursing environments, with a primary focus
on enhancing communication and swallowing abilities. This role
involves collaboration with interdisciplinary teams to provide
tailored care based on physician referrals and regulatory
requirements. Candidates must possess the necessary licensure,
certification, and experience in post-acute care settings,
prioritizing patient-centered therapy while adhering to
professional and ethical standards. Environment & Patient
Demographics Our facilities function as Skilled Nursing Facilities
(SNFs) that offer: Long-term care for residents needing continuous
skilled support and compassionate daily assistance. Short-term,
sub-acute rehabilitation for individuals recovering from surgery,
illness, or injury. Collaborative care through a team-based
approach involving nursing, therapy, and medical professionals
working in unison. A diverse patient demographic that includes
those with orthopedic, neurological, cardiopulmonary, and
post-acute needs. The patient mix may vary slightly by facility. As
a therapy professional, you will play a crucial role in helping
patients achieve functional improvements and enhance their quality
of life within a supportive, patient-focused environment. Position
Overview The Speech Language Pathologist (SLP) delivers direct
rehabilitative care in speech therapy to patients based on
physician referrals, ensuring compliance with all relevant laws,
regulations, and organizational standards. Education, Experience,
and Licensure Requirements Graduation from an accredited speech
therapy program (minimum MS or MA in SLP). ASHA certification
(CCCs) OR eligibility for a 9-month Clinical Fellowship Year (CFY)
to obtain ASHA certification (CCCs) and/or meet ASHA credentialing
criteria. Once obtained, ASHA credentialing must be maintained.
Current registration/licensure in the applicable state, with an
active license in good standing throughout employment. Preferred
one (1) year of experience in post-acute care or a related setting.
CPR certification required upon hire or to be obtained during
orientation, with the necessity to keep it current throughout
employment. Specific Job Responsibilities Engage in community
health initiatives/projects as appropriate. Proficient in Microsoft
Word, Excel, and email communication. Coordinate with patients,
families, and support departments to effectively plan for patient
needs. Exhibit proper body mechanics at all times. Demonstrate the
ability to make independent decisions when necessary. Familiarity
with practices, procedures, and regulations governing operations in
post-acute care facilities. Implement and interpret departmental
programs, goals, objectives, policies, and procedures. Perform
competently in all areas, including patient rights, safety, and
sanitation. Maintain professional relationships with colleagues,
vendors, and others. Ensure confidentiality of all proprietary and
sensitive information. Understand and adhere to organizational
policies, including those related to harassment and compliance.
Exhibit integrity and professionalism by following the
organization’s Code of Conduct and completing mandatory compliance
training. Essential Functions Establish, evaluate, and adjust
realistic, measurable, timely, and functional goals. (SLP CCC only)
Supervise and assess care provided by SLP CFYs. (SLP CFY only)
Follow speech therapy treatment plans for patients under the
guidance of the supervising SLP CCC, adhering to Federal and State
regulations. Document care accurately and in a timely manner.
Utilize therapy software effectively and accurately. Provide
excellent customer service and maintain a positive attitude towards
patients. Assist in the safe evacuation of patients when necessary.
Demonstrate reliable and consistent attendance. Employ reasoning
skills and sound judgment in all situations. Communicate
effectively and work collaboratively within an interdisciplinary
team. Engage in various physical activities such as sitting,
standing, bending, lifting, pushing, pulling, stooping, walking,
reaching, and moving intermittently during work hours. Read, write,
speak, and comprehend the English language proficiently. An Equal
