Care Manager RN - Panorama at Providence Health and Services summary: The Care Manager RN coordinates care for patients with chronic illnesses by collaborating with primary care providers and community resources to improve patient self-management and health outcomes. The role involves assessment, patient education, and promoting evidence-based, cost-effective care within ambulatory settings. The position requires a registered nursing license and experience in managing complex patients, preferably with a Bachelor’s degree in Nursing and familiarity with the Medical Home Model. Description The Care Manager RN is responsible for managing patient populations with chronic illness and/or multiple comorbidities. This individual works in coordination with multiple PCPs and serves as the central link to an identified population of patients. Through skilled assessment, education, collaboration and coordination of healthcare and community resources, the RN Care Manager Navigator assists patients to gain self-efficacy/management skills, achieve optimum functional health status, and quality of life. Assists patients/families, staff and systems to achieve high quality, evidence based, cost-effective, and patient focused outcomes. This person works under minimal supervision within the general policies and standard practices and the nursing code of ethics, including Chapter 18.88 RCW. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
