Roles & Responsibilities Process and manage insurance authorizations to keep care moving smoothly Coordinate post-acute referrals and arrange transitions to home care or facilities Deliver Medicaid notices to patients with accuracy and compassion Document thoroughly and efficiently in Epic to support continuity of care Work with internal and external team members to arrange DME and follow-up services Refer patients to rehab and long-term care resources when appropriate Support discharge planning by assessing needs and connecting resources Educate patients and caregivers to help them navigate post-discharge plans Participate in pre-cert review and utilization tasks to ensure appropriate care pathways Qualifications Minimum 1 year of relevant experience (1 year required) Epic charting experience is required — you’ll be using Epic daily MSW preferred but not required — clinical background helpful, not mandatory Washington state license is not required for this role First-timers accepted — if this is your first assignment, we’ll welcome you Travel experience is not required; locals are not accepted for this posting Completion of the 40-hour orientation is required Education MSW preferred Knowledge & Skills Solid understanding of care coordination and discharge planning Experience with Medicaid and Medicare processes and community programs Prior authorization and pre-certification workflow knowledge Familiarity ordering/assessing DME needs and benefits eligibility Strong documentation skills and attention to detail in Epic Ability to coordinate with interdisciplinary teams and external partners Comfort educating patients and caregivers about post-discharge plans Benefits Facility-paid parking — no meter hunting on your first day Guaranteed 72 hours per 2-week period for predictable scheduling 40-hour orientation so you feel confident and supported Work at a Level II Trauma Center with a collaborative interdisciplinary team Daytime schedule with built-in flexibility to accommodate life outside work Meaningful professional growth and hands-on Epic charting experience Supportive environment that values care coordination and patient advocacy Opportunities to work closely with case managers, PT, pharmacy and community resources Travel Perks: Explore Everett, WA Everett sits on the shores of Puget Sound with easy access to waterfront trails, the Everett Marina and scenic parks — perfect for after-shift strolls.
